Applicants may obtain Grant Application Brochures via written request or FAX (352) 795-1970 to the Foundation. The Grant Application Brochure (in Adobe Acrobat format) can also be downloaded from this site.

For Netscape Navigator:

Whether using MS Internet Explorer or Netscape Navigator, a dialog box will come up where you specify the location to download to. Browse to where you want the file saved to and then click OK.

The files are in Adobe Acrobat format which requires the Adobe Acrobat Reader. To download Adobe Acrobat, click the download button below.

Completed grant applications must include Forms 1-7 (see brochure).

Two completed copies of all required forms must be received at FAMSI by September 15th.  Incomplete or late applications will be returned.
